Release Date

Pathologic 3 will launch on January 9, 2026. That date is nailed down, screwed in, wrapped in commitments, and it isn’t moving.

Pathologic 3 is a complex game. It blends many mechanics: sometimes non-obvious and not fitting neatly into a conventional genre, because we built them to best express the Bachelor’s worldview and story. It’s a game that’s hard to explain with “keywords”; the truth is in the details. So instead of one trailer, we made several — each showing Pathologic 3 from its own angle. Here’s the first:

Steam Next Fest Demo

Steam Next Fest kicks off today, and we’re bringing a new free demo of Pathologic 3 – an introduction to the full game with unique content we haven’t shown anywhere before.

What’s in the Demo

This demo is a slice from the beginning of the upcoming game. Daniil Dankovsky, the Bachelor, arrives in the City-on-Gorkhon. He wants to meet an immortal man. And then everything goes sideways. If you can, please download the demo right away! Even if you’re avoiding spoilers and plan to play only the full game, consider grabbing it now. The more people who download the demo, the higher it rises in the algorithms and the more Next Fest visitors will see it. It costs you nothing but time and disk space, and it helps us a lot.

We’re excited to share that on October 13, as part of Steam Next Fest, a new free demo of Pathologic 3 will become available.

How is the new demo different from “Quarantine”? In every way! The new demo is a slice from the beginning of the full game. It has a different story, covers different events, and gives you a look at the very start of Pathologic 3. Playtime: about 1–2 hours.

What should you expect? Daniil Dankovsky, the Bachelor, arrives in the Town-on-Gorkhon. He wants to meet the immortal man. See you at the point of arrival. We’ll be waiting on the platform to help with the luggage.

The Cost of Mistakes

You step into an unfamiliar town for the first time. You meet its residents, explore its districts, learn their customs. You encounter death. You find allies and make enemies. You commit irreversible errors. You face the consequences of your choices. You bear responsibility. You live with it. This is all a fascinating process, because failure in a video game isn’t as frightening as in real life. Yet even in games, people are sometimes so afraid of making mistakes that they reload saves, read walkthroughs, watch YouTube essays or quit entirely before reaching the end. In Pathologic 3 we decided to flip that concept and look at mistakes from the other side. What if any mistake could be undone — like in the fantasies above? 12 Days Until the End

A renowned doctor from the capital arrives in a steppe town in search of immortality. Instead of an immortal man, he finds a corpse. Instead of eternal life, an epidemic of a monstrous disease. The town descends into chaos and dies in twelve days. The end. Pathologic 3 begins in the middle of that story. You’ve already made every possible mistake, taken every wrong turn. The plague tears the town apart, key figures are dead or have turned against you, and catastrophe is just around the corner. There is no hope. …And at that moment you gain the power to rewind time and attempt to save the town — by making different choices. {STEAM_CLAN_IMAGE}/45235074/4f8b81ef1eb3bcbc3e80df0952575a9fec8f7506.gif Like in previous games, you will study the town and try to save its inhabitants from what seems inevitable: examine patients, issue harsh decrees to fight the epidemic, visit plague-ridden districts. And if time corners you, you will rewind and try again. Unlike previous entries in the series, now you must fight for every new day of the town’s life, because the story keeps trying to end prematurely. You need to find a vaccine as early as possible; your diagnostic skills and medical knowledge will be crucial.

Be Ready to Answer for Your Actions

The narrative frame is: Dankovsky is interrogated about what happened to the town. He’s blamed for everything but what exactly happened isn’t always clear. Battling a plague is no child’s play. No one trusted you. People barely learned to wash their hands, yet you had to invade their private lives: impose forced quarantines, curfews, burn down entire districts, punish those who broke the decrees. Dankovsky will be accused of failures, cornered: he’ll have to change his testimony to defend himself. Confronting unexpected consequences, he must choose other paths until he assembles a version of the world that suits his will. Because, as we know, any choice is right so long as it’s willed. {STEAM_CLAN_IMAGE}/45235074/34ee2d6953c7d1beeae64435041adda67e25a971.gif We aimed for as few obviously good or bad decisions as possible. Many stakes are hidden: sometimes you only know that “the price will be something important,” but what exactly (and whether it’s worth it) will be revealed later. In a game with linear time that would feel cruel; the ability to rewind lets you explore the unknown boldly and err without fearing permanent ruin. Study the possibilities. Build theories and test them. Own the consequences.

Minutes of Life Are Worth Their Weight in Gold

Within the game world you need something called Amalgam to travel to another point in the story. It can be: • obtained by completing quests; • “taken” from those beyond saving (euthanasia); • harvested by breaking mirrors every reflection doubles life, and therefore time. (What Amalgam is and where it comes from will be explained in-game by the Kain family – if they feel like it.) Spend Amalgam and leap into the past: for example, jump from Day 5 back to Day 3, save a doomed character — and return to Day 5 in an altered reality. If the result displeases you, the updated Mind Map comes into play, letting you manipulate memory: erase an entire branch of events and try a different combination. In other words, if one decision bothers you, you can go back and change only that, leaving the rest of the day untouched — provided the events aren’t logically linked. {STEAM_CLAN_IMAGE}/45235074/b63d563bdf419c13fb5e66ec1942545c3bf02286.gif These jumps in narrative and time aren’t an optional gimmick — they’re the essence of the game. Certain endings, locations, and crucial story answers unlock only after you arrange specific combinations of events. It’s the path from hostage of circumstance to a person trying to build the perfect puzzle of occurrences. Assemble the world into a single riddle — and become something greater.

We continue gathering feedback and polishing the Amalgam mechanic, the balance of jumps, and quest overlaps. Hello. The release of Quarantine was a new experience for the studio. We’ve made demos before (The Marble Nest for example) but we’ve never gathered feedback this systematically. Hundreds of thousands of players tried the prologue, and we got to hear their thoughts: via the Steam community, on social media, and of course, in the survey. Quarantine helped us test new mechanics that will appear in Pathologic 3 and draw some valuable conclusions. {STEAM_CLAN_IMAGE}/45235074/8c55c8d9d6fbbee0bbe0649a315ab82f5bd1c178.gif Even Quarantine itself, which we don’t plan to update further, received rebalanced difficulty, QoL improvements, and several HUD variants after launch. Hospital gameplay

{STEAM_CLAN_IMAGE}/45235074/d19e4bc5165c669e56f79d8bd6188cb7a4796fe8.gif The medical detective gameplay (examining patients and diagnosing them) is clearly the highlight of the prologue. That’s obvious from both the feedback and the survey ratings. Good news: there will be a lot more of it in the main game! Of course, getting to know eccentric townsfolk and grappling with the existential drama of human boundaries won’t go anywhere, but you’ll also have to engage deeply with the tragedies and illnesses of ordinary people. We can’t even say “way more than before,” because, well, this mechanic simply didn’t exist before. In the full game, these diagnostic stories will be more freeform than in the demo (where we were teaching the player, so mistakes were practically not allowed). We’re already experimenting with ways to make symptom logging (and therefore diagnosis) more convenient and engaging.

Localization

Our plans haven’t changed: we hope to include English, Russian, German, Simplified Chinese, Italian, and Brazilian Portuguese at launch. This is the first time a Pathologic game will launch with localization beyond Russian and English. The script had a lot of words, and writing quality matters deeply to us. That makes localization hard, but we’re committed to making the game more accessible for a broader audience. If we manage to expand it further, we’ll let you know!

City traversal

The main question after Quarantine: what’s up with the city? Why couldn’t you move through it without loading screens? And how will that change in the full game? {STEAM_CLAN_IMAGE}/45235074/d6e4bf4ff6f033cf220d253ea68f062eb506828d.png …Why change a game where everything “worked”? Why not just write a new story in the same setting? We know some players would’ve liked that just fine. But for us that wouldn’t be enough. Not creatively (there’s already a game about digging through Gorkhon’s trash cans), and not commercially: sequels to indie games statistically sell much worse than the originals, and we’re making a sequel to a sequel. Just repeating ourselves? That’s a dead end. But trying new things is in the studio’s DNA. And that led us to the idea of a new kind of game — about a person lost in time, with narrative structure and space-time torn to shreds. That presentation will stay. It’s a core part of the concept — it can’t be changed. (And technically, it can’t be reversed at this point.) What will change in the full game, however, is the feel of the city. More specifically:

Route choice — this already exists, but was limited in Quarantine. Navigating the city should always feel like a small puzzle for Dankovsky.
Non-linearity — don’t forget, events will continue to occur simultaneously in different parts of the map. Even within one district, multiple things can happen — sometimes even conflicting ones.
Location content and situations — a lot more, and far more varied. Quarantine was a parade alley: a glimpse at the game, not the game itself. Many features that will be in Pathologic 3 didn’t make it into Quarantine.
City impact — the effects of your decrees will be visible. (Told everyone to wash their hands? They’ll be standing there, washing!) That’s also part of the city connection: issuing an order and seeing it come to life. It’s an important aspect of Dankovsky’s path.
Immersion… well, let’s talk about that in more detail.

Immersion

{STEAM_CLAN_IMAGE}/45235074/f9f6538485ca4f02501dc42f98d939666b023131.jpg While the Haruspex sank into the flesh of the city, the Bachelor sinks into both the city and his own mind. We believe it’s possible to create a consistent sense of presence — even without consistency in time and space. You can doubt where and when you are, and still not lose the thread of your thoughts. That sensation is our goal in Pathologic 3.

Optimization

Even over a few Quarantine patches, we significantly improved performance. But the main round of optimization for the full game is still ahead. In other words — we’re planning for things to get much better.

Apathy/mania

Dankovsky’s mental state is a new mechanic. In short: we’ve seen that the overall concept works, but it’s not yet well-balanced (we’re working on it) or well-explained (we’ve improved it a bit with patches and HUD variants, and we’ll keep working on it). By release, we aim to fine-tune this “mental metabolism” so that it’s exactly as unnerving as we want: no more, no less.

Will there be an FOV setting in the full version?

Yes.

Will the content from Quarantine remain in the full game?

Originally, we planned to use Quarantine as the game’s intro. But we now believe we can make a smoother, prettier intro that introduces mechanics earlier. So most likely, some scenes from Quarantine will show up later in the game as flashbacks — but not at the beginning. As a complete experience, Quarantine will remain separate. (Don’t worry — that means more Seraphima and Platon, more Inspector and Capital — not fewer!) We’ll also give you a heads-up: the prologue’s store page will be taken down closer to the full game’s release. Everyone who already added it to their library will still be able to play it — so make sure to grab this piece of the story while you can.

To sum up

Quarantine turned out to be an interesting and very useful experiment — one we hope will help make the full version of Pathologic 3 even better. Hey, The development of Pathologic 3 continues, and so does our conversation. Today, we’ll talk about a mechanic that is unique to Bachelor’s story — something you’ve never seen in the series before. You Are Not a Butcher

Every game in the Pathologic series tells the story of a person in a plague-ridden town. If that person is Haruspex — a self-taught surgeon who relies on intuition, a scalpel, and his knowledge of the Steppe — you get Pathologic 2, a survival-focused game where a doctor literally plunges his hands into human flesh. But if our protagonist is Daniil Dankovsky — a renowned medical scholar, an observer, an analyst, someone who only resorts to laboratory research in the most exceptional cases — then the story takes a different turn, and the gameplay must change accordingly. {STEAM_CLAN_IMAGE}/45235074/8c55c8d9d6fbbee0bbe0649a315ab82f5bd1c178.gif That’s important to us: we strive to make both the visuals and the mechanics reflect the protagonist’s subjective perception of the world and fit their story. That’s why Pathologic 3 introduces a fundamentally new approach to gameplay.

A Chance for Salvation

Finding himself in a city ravaged by the Sand Plague, Daniil, as a true scientist, notices something strange. Some patients don’t die within five hours as they’re supposed to. Could it be that… the Sand Plague is weakened by other diseases? If that’s the case, then by studying infected patients and uncovering the cause of this anomaly, one could develop a vaccine. An imperfect, temporary solution… but what if it’s the first step towards salvation? And so, from doctor and researcher, Dankovsky is forced to become a detective. You enter a hospital hastily set up in the town’s theater. Examine a patient’s medical file. Talk to them. Observe them. But something doesn’t add up. A misdiagnosis? Symptoms misinterpreted? Trust No One

Not every complaint should be taken at face value. A hand wound could be the result of a street fight, or it could be an early sign of ulceration. A patient may blame their headache on the flu, but you’ll notice symptoms of Steppe Fever. And when you come across darkened eyes, changes in pupil shape, and other anomalies, even common sense won’t be enough. You’ll need to learn to read symptoms and search for clues beyond hospital walls. A patient’s neighbors, or a forgotten item left at home, might reveal what’s been happening to them in recent days, helping you make the right diagnosis, save a life, and get one step closer to a vaccine. Staying in the makeshift hospital won’t be enough — you’ll have to search for the truth. Luckily, Dankovsky is a keen observer with a sharp analytical mind. Take Action

Of course, there’s a simpler approach: collect samples, use a microscope, and get a clear answer. You’ll see which organ groups are affected, this will be reflected in the patient’s file, and the mystery will become much easier to solve. {STEAM_CLAN_IMAGE}/45235074/8da94ee3bd24d53a8270456ff961bd174158341d.gif But… Where will you get the necessary reagents in a city ravaged by disease? Even if your research helps rule out several incorrect diagnoses… The reagents will be depleted, and the next patient might not be so lucky. Every day, new patients will come to you. If you manage to save them all today, you’ll be one step closer to developing a vaccine for the Sand Plague. But the disease mutates, and a new vaccine is needed every day. Prepare for Consequences

But… Put yourself in the shoes of an ordinary townsperson. Would you agree to be injected with an untested vaccine during an epidemic? And that’s assuming you even understand what a vaccine is — an uneducated factory worker might not. And if people are subjected to unexplained injections every day, they will start forming conspiracy theories and even riot. Be careful when issuing city-wide mandates and enforcing quarantine measures. Frightened citizens won’t accept overly strict rules — if you push too hard, they will come for you and kill you before you can save the town. Unfortunately, a doctor from the Capital must fight not only the plague and death itself but also his own patients. To Kill Death

Sometimes, exhausted Dankovsky closes his eyes and thinks: if only death could simply be killed. The locals believe the plague is brought by a Steppe golem — Shabnak, a monster from legend. If it were real, if he could shoot it with his fancy Capital-made Prototype gadget, he would take the risk, no matter how impossible it seemed. Q: What happened to the route for Pathologic 2? Has it evolved into a sequel? A: Yes, that’s exactly what happened: the game simply grew beyond our concept of DLC—it will have different gameplay and entirely new features. Instead of one game with three playthroughs using the same gameplay but different storylines (as in the original 2005 release and the Classic HD remaster), we’re creating three fully developed games with distinct gameplay and significant differences in narrative. Now, these are truly three separate protagonists with unique worldviews, perceptions of reality, and distinct tools. From a gameplay perspective, this means three different approaches and a complete rethinking of the original scripts. One storyline was released in 2019, centered on the Haruspex. It was a large game that took much more time and work than initially planned. For publishing reasons, it was titled Pathologic 2. Pathologic 3 is a new, full-fledged game focused on the Bachelor. The title was chosen to avoid confusion, making it clear that this is not a DLC but a complete game with a similar playtime. Q: What about the Changeling? A: Finally, there’s the Changeling, but we’d prefer not to make promises prematurely, so we’ll return to her when there’s more to share. Moving forward, we plan to stick to this approach. We understand that premature promises about the previous Pathologic led to unfulfilled expectations. We apologize for that and don’t want to mislead you in the future. The developer should not be an unreliable narrator—unlike his characters. Q: How do Pathologic 2 and Pathologic 3 differ? A: The protagonists! And that’s only half-joking. The main idea and reason for dividing these games is that the Haruspex and the Bachelor are very different characters. The further we developed the Bachelor’s story, the clearer this became. We didn’t want to compromise by making the game a “safer” rework of the original script, so we reimagined the core gameplay loop: The Haruspex needed to survive, fight, scavenge, rummage through garbage for scraps of bread, and try to save those he held dear along the way. The Bachelor is a man of intellect. Survival is not his primary concern; his goals go beyond what one lifetime could contain. He needs to think, formulate theories, and gather evidence. Ultimately, he must issue commands and observe the results. This distinction will be emphasized through gameplay, mechanics, objectives, soundtrack, and so on. After Save and Sound, we plan to return later this year to show you what we mean. As they say, ""Show, don’t tell."" Q: Who will receive game keys? A: We remember our commitments to those who supported us during the fundraising stage nearly ten years ago, so we will provide keys to:

He did a great job translating 250,000 words from English! «In translating the game, I created a variety of variants and studies to make an accurate translation of the characters' names. Each character has its own style of speech, its own way of life, and their names are as important as their personalities. I tried to convey all of this in Portuguese while preserving elements alien to us, because it is obviously a game with its atmosphere and many elements from Eastern Europe and Russia as a whole. Pathologic 2 is supposed to feel almost unbearable. However, we concede that every player has their own limit. As you can see, we also explain why we believe it is a good choice to just play the game as it is—doing our best to communicate the intent while also providing options. If you toggle “Intended Difficulty” off while keeping all parameters the same, it won’t count. Want to have this yellow thing on your profile? Don’t touch the difficulty settings at all. If you’ve finished Pathologic 2 already (or are close to completion), just load the latest save you have and play the game to the ending credits, and you’ll get the achievement.

Patch 4 Notes

decreased the size of loaded textures;

minor improvements to the FPS when traversing the Town.

4. Animation Fixes:

at low stamina, knives and scalpels now perform a normal attack instead of a strong one;
polished animation switching across the whole game.

5. Visual Fixes:

taught crows to conceal their low-poly nature by not landing close to the player;
minor collision fixes;
minor fixes of Murky’s model materials;
minor fixes to lighting on interior furniture;
fixed the lighting during a number of pantomimes;
fire particles are now emitted from the largest part of a burning person (or object);
fixed the glitch that made the camera jerk around when going outside.

6. Audio Fixes:

invisible cats meow at a different rate at Notkin’s Castle now;
fixed the bug that excluded certain sounds (like fires or the hum of the Polyhedron) from audio settings;
the Abattoir speaks to you now.

7. Misc:

(NB!) all keys are fully rebindable now!
tweaked some hunger parameters slightly (VERY slightly);
fixed an error with aiming mode lingering when a firearm was put away;
typos.

Version 1.2.24644-rel-dev-st Technically this is the third patch, but the previous two were more like hotfixes, while this is a Big One. This patch mostly focused on logical and quest-related errors. We’re still working on improving performance, stability, memory usage, and fixing spikes, and you’ll see some results next week. Although some memory-related issues were fixed in this patch already! Here are the patchnotes: 1. Optimization - improved memory usage (loading and relaunching the game now requires less memory); - lowered the amount of memory required by the shader cache; - lowered video memory usage. 2. AI Fixes: - improved corpsmen behavior a bit; - plague clouds are now better at despawning; - NPCs react faster when someone attacks their allies; - fixed soldier animations (they correlate better with their behavior patterns now); - fixed NPC behavior upon surrendering; - improved NPC navigation; - fixed the lack of fighting animations for some NPCs in rare quest-related situations. 3. Quest and Logic Fixes: - (NB!) crucial items no longer have a chance to disappear upon the player leaving the Abattoir; - fixed an unfortunate bug that made all items disappear from the player’s hideout; - fixed the start and end conditions for some quests (e.g., Saburov will no longer try to capture you after he’s done with trying to capture you); - fixed the issues with the quest about Father’s house; - in the quest that has you barter with the townsfolk, the game now correctly displays the fact that it improves your reputation; - some quest-related items now properly disappear when their time is due instead of lingering in the game world; - fixed some unfortunate phrases and name errors; - fixed the bug that made some doors lock on you in quest-related situations; - fixed some dialogue condition errors; - fixed the wrong thoughts (and in the game). 4. Animation Fixes: - NPCs are now better at following the player with their faces and eyes; - minor fixes to women’s movement; - minor fixes to facial animations; - more natural poses in some dialogue animations; - soldiers’ aiming is no longer jerky. 5. Visual Fixes: - burned people now produce prettier smoke; - some characters no longer float above their designated places (happy landing); - improved the shadows on Aglaya’s face; - improved the look of fire (and made it less bright); - minor collision fixes; - fixed visual glitches on fences; - improved the shadows cast by table lamps; - the lighting in the Lump has become slightly prettier; - improved the overall quality of indoor lighting, especially through doorways; - fixed the village quay; - now some detailed models load from further away (making the game a bit prettier); - fixed jerky camera movement; - partially fixed the incorrect blood particles upon hitting the opponent; - the grass now looks more even from afar; - fixed the look of the crosses drawn on doors; - fixed the blinking tooltips on the map (happened when the cursor was placed on a character portrait); - fixed trembling dialogue options (it was a bug, not a compelling storytelling technique). 6. Audio Fixes: - audio balance generally improved; - restored music to the Crucible (Victor’s wing); - improved revolver and rifle sounds; - added music to the Nutshell. 7. Misc: - made a single notch of the gamma slider twice as small, meaning it’s now easier to fine-tune it; - added VSync options to the graphic settings, that allows to set the FPS to ½, ⅓, or ¼ of you display refresh rate (improves performance at the cost of FPS); - fixed the bug on the “Load Game” screen that made the Load and Delete Save buttons active even in the absence of a valid save; - stealth is now active for the duration of the button being pressed, not toggled; - typos. Please keep in mind that patches have a time lag: not all errors that we’re aware of have already been fixed. But whatever is not in this patch, is coming soon in future ones!

Pathologic 2 was always intended to be gruelling, stressful, and bleak; we believe in ludonarrative cohesion and aren’t too fond of stories that are only dark and hurtful on the cover. However, we also believe—even firmer—in player freedom. Pathologic 2 was balanced around three principles:

We want the player to always balance on the verge of death, but also always to have an opportunity to drag themselves out (perhaps loading a save or two back—that’s why the game keeps the full history of your saves)
We want the player to be forced to sacrifice “cool and interesting” content due to their survival needs, to be always short on time and not being able to achieve an optimal playthrough. The story of Pathologic 2 is written to be caught glimpses of, not unfold completely
We want the player to do things that feel clearly wrong, like voluntarily thrashing their reputation or using the healing items that could have saved someone. Among other things, Pathologic 2 is an exploration of selfishness—that video games often allow us to completely disregard as a factor

Judging by your feedback, Pathologic 2 is actually balanced in a way that achieves all that. However, we have received clear feedback that the game is too hard—and we don’t think that everyone who says that misses the point of the experience. People are different and have different attention spans and patterns. So we’re fine with allowing the players to tweak the game a bit to account for this fact. Our games have always been in a curious spot. Some people only finished the original Pathologic and The Void using cheat codes. So while in theory these players agreed with our stance on games as a medium for creating compelling experiences, in actuality they didn’t really—well—experience what we had in store for them. Which is, of course, understandable. Not everyone wants a life-changing experience every time they launch a game. Sometimes people only want a cursory glance. Well, we’d rather give people a tweaked experience than none at all. So, in the coming 2-3 weeks we’ll introduce a difficulty slider to Pathologic 2. You’ll be able to tweak the game mildly, within the limits of what we consider intended difficulty, and also set it the way you want, if you want. But we hope that you don’t. Pathologic 2 is supposed to be almost unbearable, otherwise the effect is lost. We concede that everyone has their own limits to push. But we strongly advise against making the game easy for yourself. However, we do like the notion of giving you this freedom—and this responsibility. This way, the achievement of resisting the temptation and finishing the game on intended difficulty becomes even more true and vivid. And that’s the kind of effect we deeply appreciate.

The first patch is here! It won’t be the last—a meatier update awaits ahead, while this one is really more of a hotfix. Here’s the changelog:

CONTROLLER SUPPORT (kinda) Pathologic 2 supports a controller, but the controls themselves are unrefined yet. We turned it off for now, because we can’t guarantee playing with a controller would be a sufficiently good experience. However, those of you who want to experiment can do that now by turning the controller on in the game files. To do that, find C:\Users\username\AppData\LocalLow\Ice-Pick Lodge\Pathologic 2\Settings\ExternalInputSettings.xml and set true. Do be warned though that we’re still honing this aspect though.
